In recent years, there have been concerted efforts to better recruit, support, and retain diverse faculty, staff, and trainees in academic medicine. However, many institutions lack comprehensive and strategic plans to provide support to retain and recruit individuals from historically underrepresented groups. In this article, we itemize specific mechanisms through which institutions can support diverse individuals with the goal of improving inclusion and belonging in the workforce to better reflect the diversity of the intended patient and research participant population.

Objectives:
The European Prediction of Psychosis Study (EPOS) aimed to study a large sample of young patients who are at risk of psychosis and to estimate their conversion rate to psychosis during 18 months follow-up. This presentation describes quality of life and its changes in patients at risk of psychosis.
Methods:In six European centres, 16 to 35 year old psychiatric patients were examined. Risk of psychosis was defined by occurrence of basic symptoms, attenuated psychotic symptoms, brief, limited or intermittent psychotic symptoms or familial risk plus reduced functioning. Quality of life (QoL), measured by the Modular System for Quality of Life, was assessed at baseline and at 9 and 18 months’ follow-ups. Psychiatric patients without prodromal symptoms and healthy subjects were comparison groups.
Results:In all, 245 risk patients were included. At baseline, they reported lower QoL than non-risk patients and healthy controls. Basic symptoms associated negatively with QoL, and there were differences between the study centres. During the follow-up, QoL raised less in risk patients than in non-risk patients. Baseline QoL did not predict transition to psychosis. However, its development was poorer in patients with than in those without transition to psychosis.
Conclusions:Those of the psychiatric patients who are at risk of psychosis have lower QoL than other psychiatric patients or healthy controls. QoL does not predict transition to psychosis, but its changes correlates with changes in clinical state. The results indicate that there is a need for comprehensive intervention with the patients at risk of psychosis.

Introduction: A two-year Stroke Ambulance (SA) pilot project was implemented at the University of Alberta Hospital (UAH) in February, 2017, the first in the world to utilize this specialized technology in a rural setting. The primary objective is to evaluate clinical and economic implications of timely SA assessment and treatment of hyperacute stroke patients who present to non-stroke centres in rural Alberta and might otherwise have received delayed treatment, or not at all, due to prolonged transfer times. Methods: A steering committee and seven working groups were established, with representation from Alberta Health Services (AHS) programs impacted, to ensure comprehensive project development and implementation. The SA portable CT scanner, point of care laboratory, and videoconference system facilitate diagnosis of stroke in the field. The multidisciplinary team includes a stroke fellow, advanced & primary care paramedics, registered nurse, CT technologist, and telestroke physician. When not dispatched, the team provides stroke expertise and patient care in the emergency department (ED) and diagnostic imaging. The service model includes suspected stroke patients presenting to non-stroke centres within a 250 Km radius of Edmonton (Phase I); patients presenting to Edmonton Zone (EZ) hospitals without CT capability and/or tPA protocols (Phase 2); and expedited transport from EZ hospitals to the UAH for urgent endovascular therapy (EVT) (Phase 3). A health economic analysis will compare stroke ambulance care with standard care. Results: The SA has responded to 54 dispatches, 13 patients thrombolyzed and 3 patients receiving EVT. Median rendezvous to CT time was 10 minutes, median rendezvous to tPA time was 21 minutes, and mean time from symptom onset to tPA was 180 minutes. There were no complications. After SA imaging and assessment, 18 patients were repatriated back to their local community hospital, avoiding unnecessary admission to tertiary care. Conclusion: Our preliminary experience demonstrates that the SA offers a novel approach to performing timely evaluation and treatment of suspected stroke from non-stroke centres and may serve as an excellent triage mechanism, reducing avoidable admissions to overcapacity tertiary care EDs. The SA team provides added value to the ED with stroke expertise and patient care. A comprehensive health economic analysis will determine cost-effectiveness and whether spread is feasible.

Field studies were conducted to determine suitability of flurtamone alone and in tank mixture with ethalfluralin for selective control of weeds in safflower. Flurtamone at 0.6 to 0.9 kg ha−1 controlled field pennycress, kochia, and Russian thistle. Flurtamone and ethalfluralin were compatible in tank mixture and provided comparable or superior control of wild oat, kochia, and field pennycress than either herbicide alone. Plant height, date of flowering, seed yield, and quality data indicated that safflower tolerated flurtamone alone up to 1.6 kg ha−1 and flurtamone tank mixed with ethalfluralin at rates as high as 1.1 plus 1.1 kg tar1, respectively. Safflower yield response to these herbicides was positive and reflected level of weed control attained with various treatments.

Epidemiology formed the basis of ‘the Barker hypothesis’, the concept of ‘developmental programming’ and today’s discipline of the Developmental Origins of Health and Disease (DOHaD). Animal experimentation provided proof of the underlying concepts, and continues to generate knowledge of underlying mechanisms. Interventions in humans, based on DOHaD principles, will be informed by experiments in animals. As knowledge in this discipline has accumulated, from studies of humans and other animals, the complexity of interactions between genome, environment and epigenetics, has been revealed. The vast nature of programming stimuli and breadth of effects is becoming known. As a result of our accumulating knowledge we now appreciate the impact of many variables that contribute to programmed outcomes. To guide further animal research in this field, the Australia and New Zealand DOHaD society (ANZ DOHaD) Animals Models of DOHaD Research Working Group convened at the 2nd Annual ANZ DOHaD Congress in Melbourne, Australia in April 2015. This review summarizes the contributions of animal research to the understanding of DOHaD, and makes recommendations for the design and conduct of animal experiments to maximize relevance, reproducibility and translation of knowledge into improving health and well-being.

Governing the urban wetlands: a multiple case-study of policy, institutions and reference points

Worldwide, coastal and floodplain wetlands are rapidly urbanizing, making them highly vulnerable to biodiversity loss, biological invasion and climate change. Yet urban wetlands management is an understudied area of global environmental research. Different policy approaches and institutional arrangements in place for urban wetlands governance have to be studied comparatively to obtain a better understanding of the current issues. This paper investigates four urban wetland policy regimes and the application of ecological reference points across four countries. The regimes are discussed within the context of global policy trends, urbanization patterns and environmental change. The analysis illustrates that the four cases deviate substantially in certain characteristics and converge in others. Global trends such as environmental treaties and restructuring of city spaces are common policy drivers for all cases. Conversely, the localized specific problems have yielded specialized policy responses in each case. Declaration of fixed biological reference points for wetlands were not used at any stage of the policy development process. However, the wetland managers formally or informally set up ecosystem-services oriented benchmarks for urban wetland management. Globally-applicable normative policy directives or universal ecological reference points seem bound to fail in urban wetlands governance. However, in designing effective urban wetland policy and institutions at the regional scale, both context-specific and generalized lessons from empirical policy evaluation of multiple case studies need to be jointly considered. Based on the characteristics of the policy regimes analysed in this study, a hypothetical framework for urban wetland policy evaluation is proposed; this has yet to be validated by empirical application to actual cases.
Cognitive therapy for people with a schizophrenia spectrum diagnosis not taking antipsychotic medication: an exploratory trial

Background
Although antipsychotic medication is the first line of treatment for schizophrenia, many service users choose to refuse or discontinue their pharmacological treatment. Cognitive therapy (CT) has been shown to be effective when delivered in combination with antipsychotic medication, but has yet to be formally evaluated in its absence. This study evaluates CT for people with psychotic disorders who have not been taking antipsychotic medication for at least 6 months.
MethodTwenty participants with schizophrenia spectrum disorders received CT in an open trial. Our primary outcome was psychiatric symptoms measured using the Positive and Negative Syndromes Scale (PANSS), which was administered at baseline, 9 months (end of treatment) and 15 months (follow-up). Secondary outcomes were dimensions of hallucinations and delusions, self-rated recovery and social functioning.
ResultsT tests and Wilcoxon's signed ranks tests revealed significant beneficial effects on all primary and secondary outcomes at end of treatment and follow-up, with the exception of self-rated recovery at end of treatment. Cohen's d effect sizes were moderate to large [for PANSS total, d=0.85, 95% confidence interval (CI) 0.32–1.35 at end of treatment; d=1.26, 95% CI 0.66–1.84 at follow-up]. A response rate analysis found that 35% and 50% of participants achieved at least a 50% reduction in PANSS total scores by end of therapy and follow-up respectively. No patients deteriorated significantly.
ConclusionsThis study provides preliminary evidence that CT is an acceptable and effective treatment for people with psychosis who choose not to take antipsychotic medication. An adequately powered randomized controlled trial is warranted.

The prevalence, distribution and severity of detectable pathological lesions in badgers naturally infected with Mycobacterium bovis

The Randomized Badger Culling Trial (RBCT) began in 1998 to determine the impact of badger culling in controlling bovine tuberculosis in cattle. A total of 1166 badgers (14% of total) proactively culled during the RBCT were found to be tuberculous, offering a unique opportunity to study the pathology caused by Mycobacterium bovis in a large sample of badgers. Of these, 39% of adults (~6% of all adults culled) had visible lesions (detectable at necropsy) of bovine tuberculosis; cubs had a lower prevalence of infection (9%) but a higher percentage of tuberculous cubs (55·5%) had visible lesions. Only ~1% of adult badgers had extensive, severe pathology. Tuberculous badgers with recorded bite wounds (~5%) had a higher prevalence of visible lesions and a different distribution of lesions, suggesting transmission via bite wounds. However, the predominance of lesions in the respiratory tract indicates that most transmission occurs by the respiratory route.

In Stockwell and Scott's commentary on our papers, in which we concluded from a significant mass of data (a fraction of which was presented in the paper) that we questioned the notion of ‘global patterns’ of matrix distributions within articular cartilage. In part, this was based on the distribution of keratan sulphate and chondroitin sulphate within the knee (both tibial and femoral compartments although only the latter was illustrated in the paper when referring to adult animals) of our model, the marsupial Monodelphis domestica.

The material and spectroscopic properties of Cr-doped Ca5(PO4)3F (FAP) were investigated using several different chemical and physical means. From analytical titration measurements the chromium in Cr:FAP was found to be tetravalent. ESR measurements detected a trace (0.5 - 10 ppm) of Cr5+ (3d1) and no evidence for Cr3+ (3d3). The absorption and emission spectra of Cr: FAP can be interpreted on the basis of Cr4+ (3d2) ions substituting for P within a distorted tetrahedron of oxygen ions. Calculated electronic (Stark) levels for Cr4+ based on an approximate site symmetry of C4 agree well with levels deduced from an analysis of the experimental optical data. The model Hamiltonian includes Coulombic parameters B and C, interconfiguration interaction terms and the spin-orbit coupling for the 3d2 atomic configuration of Cr4+ and crystal-field terms in C4 symmetry. The parametrized Hamiltonian was diagonalized within the complete d2 configuration. Both free ion and crystal field parameters are presented.

Combination of SAXS and USAXS measurements provide an extended q-range (0.006–3.0 nm-1) to study fractal growth of both aging silica gel as well as precursors of zeolite-A. Mass (silica) and surface (zeolite) fractals are observed. Scanning transmission X-ray microscopy (STXM) proves to be an extremely useful technique to obtain direct images of wet samples in the 0.1–100 micron range, confirming the SAXS/USAXS results on even larger length scales.

The physical study of planets and satellites is probably one of the more active fields of research of the second half of this century. This is due to space exploration by spacecraft, but also to the use of modern detectors, of large ground-based telescopes, and of powerful computers by active researchers. Planetary research (or planetology) is a pluridisciplinary domain, which requires not only the competence of astronomers, but also of geophysicists, of mineralogists, of climatologists, of biologists, of chemists, of physicists, of “pure„ mathematicians, and many other scientists. Many results are at the boundary of those of other commissions such as the 15, 20, 7, 19, 33, 40, 44, 49 and 51 ones. The study of the main results obtained during this last triennum shows a perfect complementarity between space and ground-based observations. It should be arbitrary to separate space and ground-based scientists. The have the same goal and they study the same objects. Quite often, the same individuals use both techniques, depending on the most efficient one for the problem under study. It is remarkable to see that space data collected more than ten years ago are still analysed in connection with ground-based observations. The same remarks can apply for ground-based data. In addition to that, new theoretical models, new numerical simulations and new laboratory experiments have ben recently developed. They all contribute to a better understanding of planets and satellites physics.

Since the last General Assembly in Patras, Greece, we have held three meetings of the Working Group. The 10th Meeting was held in Mzkheta, the ancient capital of Georgia, USSR, hosted by their Academy of Sciences on April 3-7, 1984. All members except one, who was represented by a member of his Task Group, were present at the very productive meeting.
